What is Case Management?
Geriatric case management is provided by professionally trained care managers who are responsible for planning, searching for, and monitoring senior citizens’ care services. Case management staff are also responsible for regularly communicating with their clients’ caregivers, families, and personal network.

How Do Case Managers Help Senior Citizens?
Many of the elderly people we work with require care while recovering from medical issues. Others need support so that they can better cope with psychological issues, or they might need caregiving assistance to compensate for a lack of a personal support network.
A case manager can help senior citizens by arranging fundamental services for in-home care or grocery shopping needs. They can also arrange services for complex situations such as transitioning to a new living situation or recovering from a car accident.

Whether our care managers need to provide an update through a written medium, or over a phone call, they are always there to keep our client’s network up to speed.
Our care managers can also provide intervene during crises, find solutions, advocate for the client in family or medical situations, provide monitoring services, and assist with many other needs. Some of these needs are seen below (but are not an exhaustive list):
• Educating clients about their diagnosis

• Incorporating physician recommendations into the care plan
